BOOKINGS TERMS & CONDITIONS

 

 

Thank you for booking an appointment with Stephanie Pittam, Registered Osteopath. If you do not agree with these terms, please do not request my services.

  1. Appointment Bookings:
    • Bookings can be made via phone or email.
    • Booking confirmation will be sent to you via email or text at the time of booking.
    • You will also receive a confirmation 2 days before your appointment.

 

  1. Fees
    • You agree to pay the appointment fees as stated on the website, plus any travel fees detailed at the time of booking.
    • Payment details will be given at the time of booking.
    • Fees can be paid preferably via Card (human clinic only) Paypal, BACS, Cash, Cheque.
    • Fees are to be paid either prior to the appointment, at the appointment or WITHIN 24 HOURS of the appointment.
    • A surcharge of £10 per day is applicable for late payments.
    • Non-payment within 30 days may result in court action.

 

  1. Cancellations
    • Please give me plenty of notice if you wish to cancel or rearrange.
    • Appointments cancelled within 48 hours of your appointment time are charged at the full fee
    • No-shows or forgotten appointments are charged at the full fee.
    • I reserve the right to cancel or change your appointment if I suffer personal illness or transport problems.
